Medical

Hilltown Community Health Center uses a patient-centered model of care to provide primary medical care to our patients at three locations.

At our main locations in Huntington and Worthington, your team (a physician or nurse practitioner, nurse, medical assistant, and receptionist) can connect you with the care you need and help you achieve your health goals while encouraging your engagement in your health care. We also have a School-Based Health Center at Gateway Regional Middle and High School so students have easy access to high quality primary care services.

At all of our locations you can find primary preventative and acute care for people of all ages, routine physical exams and DOT physicals, immunizations, pre-natal care, behavioral health and nutritional counseling, gynecological exams, laboratory, podiatry services and more. Our providers have specialty focuses that include HIV, diabetes, and other chronic disease management, women’s health, pediatrics and public health. Two of our nurse practitioners speak Spanish and interpretation services are available in any language at your request. Our facility is welcoming and inclusive. We will see anyone regardless of their ability to pay for services, and we also have trained staff that can help you get affordable and comprehensive health insurance coverage.
